Homemaker and Extraordinary Cleaning
Wells Healthcare Foundation Agency provides Homemaker and Extraordinary Cleaning services that allows the person with a disability to receive support to ensure their home environment is clean, healthy and safe. Extraordinary Cleaning and Homemaker services are available to members on the Children’s Extensive Support (CES) Waiver, Community First Choice (CFC) and the Supported Living Services (SLS) Waiver.
Services
Individual Residential Services and Supports (IRSS)
- Provides 24/7 daily support to persons with disabilities living in a Personal Care Alternative in their own home, a home owned by the agency or a host home.
- Provide nursing support
- Medication Administration
- Personalized Care Plans
- Transportation to Medical Appointments
- Access to Community Services (Parks, Theater, Library, Malls, Museums, Gyms)
- Manage resources, track receipts and keep an active record of all personal items
- Develop and implement an Individual Behavioral Services and Supports Plan as needed
- Ensure individuals are educated and understand their rights
- 24/7 on call support
Services
Supported Community Connections (SCC)
- Provide support to individuals with disabilities opportunities to build relationships and natural support in the community. SCC services are designed to support individuals with opportunities to build relationships in the community and give them experience that will enable them to access their community confidently and safely.
- Personalized Plans
- Support interactions with community members
- Assist individuals who may require behavioral support
- Educate individuals with disabilities about how to access and utilize typical community settings
- Promote safety and independence in the community
- Work on personal goals
- Volunteering Experience
- Recreational Activities
- Employment Opportunities available to people with disabilities in the community
- Educate them of their rights
- Provide SCC on a one-to-one basis or in a group setting
- Assist individuals to have as much control over their personal finances
- Educate and expose individuals to money management skills
- Provide transportation to social activities and employment activities in the community
